We have encouraged the PACE program for all of our hospital and community-based providers in Northern Manhattan. As the Medical Director of the Harlem Children's Zone Asthma Initiative, and a PACE educator, I have a vested interest in ensuring that my community colleagues are all delivering the highest level of clinical care.

| Ben, Any advice on successful approaches for PACE? Are hospitals teaching it at Grand Rounds or as a weekend seminar?

| Our asthma program is similar to PACE in that it provided asthma education to the providers. Our program goes out to the Primary care providers and their office staff to empower them to diagnose, treat and reinforce asthma education at each level within the office. Our program provides up to 6 CME/CEU's for completion and provided over lunch time or any other time the office suggests. WE have pre/post tests as well as satisfaction surveys of the asthma information. We have found as our program expanded into Kansas, that many providers have not utilized the asthma guidelines or written asthma action plans. We encourage the self management of the asthma patient so that patients will not utilize the ED and hospital setting. We still find barriers in that providers find asthma complicated and don't have time to educate their patients due to time constraints. One of the main goals with the program is empowerment of the office staff so that they can assist the provider in education for the pateint or reinforcement of what the provider has said. Though we do not have a magic wand, we are finding this approach (slow but sure) is working. PCPs have many guidelines to adhere to so important to share quick and easy approaches to how best to manage asthma. Candace Ramos,RRT, AE-C

Hi Candance, The Latino Health Initiative Asthma Program is an educational program aimed for Latino parents of children with asthma. After 8 educational sessions they improve their knowledge and self-efficacy to manage their asthma children; however we found some difficulties related the home environment and the use of national guidelines. We also found that primary health providers have no utilize the asthma guidelines or written asthma action plan. We have planed to educate the providers and your experience will be a valuable resource to for us. Would you please provide us more information about your program? Do you have a curriculum for that? We are preparing to embark on our 2nd Annual Asthma Rural Tour. Last year a staff of RTs, AE-Cs, provider, etc. went to 4 rural areas to deliver school nurse workshops, provider workshops, and a community asthma screen. While the program went well for the providers, we did not have attached CMEs. This year the program has expanded to 6 locations and we are offering CMEs.
Interestingly, the providers in Albuquerque were less interested in CMEs than the rural providers. The rural providers indicated a great need for the program and CMEs.

| Luis and Gerri, We are lucky that we are funded by a medicaid HMO-Children's Mercy Family Health partners. As a disease management program, we are able to provide asthma management programs to contracted providers within the CMFHP network. Our curriculum includes 6 modules presented that provide CME/CEUs for providers and office staff. OUr modules include the 4 core components of the updated national guidelines; 1) Strategies in asthma control, 2) Measurement and Assesment (evaluation and spirometry), 3) Pharmacological therapy 4) Envioronmental control of asthma 5) co-morbid conditions and referral to a specialist 6) Current trends in asthma (this year we are presenting bronchiolitis-last year it was VCD) After week 2 or 3, we spend 1/2 days in the office working along side the providers and staff enhancing or reinforcing what we have discussed in the didactic session. I think this is the main or one of the successes of the program. It allows for questions and answers, teaching of devices, etc, We also provide asthma action plans in triplicate to our participating offices: one for chart, one for patient, one back to us to enter into a database. We have asthma specific health coaches that work with our members with regards to medications, disease management, increased utilization, etc. We currently are seeing a rise in the ER usage, so checking into this trend to see if patients are truly using the ED more, or if we have a claims/billing glitch. Anyone else seeing an increase in ED usage??? We have many ED's in our area that are promising 30 minute guarantee's to get in and out. Much better than making a doctors appt and having to wait.
