How has your program incorporated "Reduce Impairment and Risk" into your program's goals and objectives?

I think the new approach EPR-3 takes to asthma control gives health care providers the perfect opportunity to educate patients about chronic disease management.  We all know that even patients with "mild" asthma can have a serious, life-threatening episode if asthma is not managed everyday with proper medications and trigger avoidance. 

EPR-3 gives providers and patients specific goals related to prevention and maintainence regarding impairment and risk. 

I am interested in hearing how programs plan to use tools like the patient self-assessment sheet provided in EPR-3 to assess current impairment and future risk.

One of key things our asthma program has accomplished is updating our asthma action plan. THough it is difficult to accomplish everything with one plan , we hope to include daily management,step up plan for asthma triggers and what to do when having an asthma episode. We make our action plans in triplicate so one can go to the patient, one in the chart and one back to us to enter into a database. This helps us see what is being written and and our asthma health coaches are able to reinforce the action plan while working with them.
