My program is wanting to develop a registry for our patients and we are wondering what items are important to include within this registry?

Hi Lynn ~ we have an asthma registry here at Cambridge Health Alliance, Cambridge, MA. I would be happy to talk with you by phone if that would be helpful as you set up your registry. The elements in our registry include: •Medical Record Number •Patient Demographics (name, age, gender, phone #, address, insurance, etc) •Personal Best Peak Flow •Dates of last and next scheduled visit to the Primary Care Provider •Names of Primary Care Provider and other Care Team members •Asthma Severity Classification •Does Patient have an Action Plan (Yes/No) •Medication information (start date, end date, resuce, controller for persistent asthmatics) •Dates of Asthma ED visits and Hospitalizations •Primary and secondary diagnosis of ED visits and Hospitalizations ••Date when patient was entered into the Asthma Registry
