We track key program outcomes through collecting data and evaluating our different organizations. For the Home Asthma Response Program, we track program outcomes from collected data from home visits as well as interviews with Certified Asthma Educators and Community Health Workers who are providing the intervention. HARP data are entered and stored into a secure database. Key program outcomes are analyzed and reported to our stakeholders and the CDC.
