In search of survey tool

I'm looking for a low literacy survey tool about envirnomental asthma triggers that incorporates questions on environmental justice and climate change. Anyone know of one?

As a volunteer with US Green Building Council I do a talk on residential IEQ/IAQ (do's and don't do's) and I have a "Low-Hanging Fruit" spreadsheet that goes with it so people can prioritize projects in their home. This isn't exactly what you're looking for but may possibly be a starting point. I'll email it to you and to anyone else who is interested. Karen Benson
