The Virginia Asthma Action Plan is a document orginally developed as the Southeastern Virginia Asthma Action Plan by the Consortium for Infant and Child Health (CINCH). The plan is updated yearly and is intended for use for school age children. Schools and physician practices can print and make available the form.
