Decrease ED and admissions

Children's Hospital Boston Community Asthma Initiative targets children age 2-18 who visit Children's Hospital as a result of asthma or an asthma-related complication, conducts an assessment of the family's needs for asthma care, then provides support, including asthma education, housing or legal advocacy, pest management, care coordination, and referrals to other social agencies.

The Community Asthma Management Program (CAMP) is a collaborative formed by Our Lady of the Lake RMC. OLOL physicians, nurses, social workers and respiratory therapist have partnered with EBR school system and community physicians to provide comprehensive, disease specific resources and education to asthmatic patients and their families.

The Community Asthma Network provides asthma education and awareness programming, including green cleaning programs and materials.

CAP-W teaches parents, children, educators and health providers about asthma and how to successfully manage the disease.

We provide asthma education classes in English and Spanish to the parents and child with asthma.

Community Care of Wake and Johnston Counties is a group of community nurse case managers serving Carolina Access medicaid clients that conduct home visits and environmental assessments to patients with asthma.

Community Care Partners of Greater Mecklenburg County is a Medicaid Disease Management program that helps patients navigate the medical system and conducts community education programs.

Community Case Management provides policy and program information and analysis for asthma clinics.

Community Clinic, Inc. (CCI) is a nonprofit, community-based health care agency serving residents of Montgomery and Prince George's counties who are uninsured or under-insured and others. Community Health Care, Inc. is a new asthma education program for patients that focuses on improvement of home environments to reduce missed school days and the use of rescue inhalers.

Community Health Center is a federally qualified health center serving the residents of New York City that works with the Department of Health to assure adequate screening for asthma and education of all patients with asthma and their healthcare providers.

The Pediatric CLEAR (Community Linked Educational Asthma Resource) Program commenced in May 2011 to incorporate the Expert Panel Report 3 Asthma Guidelines into the care of pediatric asthmatic patients seen in our Family Health Center (FHC) primary care clinic. This family-centered, interactive approach provides culturally & linguistically appropriate services to combat asthma in the low-income patients served by clinic., Implementation of evidence-based guidelines for identification, assessment and management of asthma in our pediatric primary care clinic has resulted in a 96% reduction in asthma-related emergency department visits over the first year, which was sustained through the second year.

Education & Prevention Network

Through the work of its Re-entry Coordinator, PCCD works to ensure linkages and collaboration with community and faith-based service providers to county planners around re-entry efforts.

We evaluate and treat pediatric asthma. We offer complete evaluation and treatment working with each client individually to control their disease.
