The New York City Asthma Network (NYCAN) is a coalition of local service providers, public health professionals, healthcare institutions, academic institutions, non-profits, and community-based organizations working together to build a coordinated asthma service delivery system and advance health equity by reducing asthma-related disparities across New York City. Through strategic collaboration and by improving care coordination and substandard housing conditions, organizing community-facing events and educational activities, and compiling useful resources for individuals with asthma and their families, NYCAN aims to strengthen the delivery of asthma care, address environmental and social determinants of health, and empower communities most impacted by the burden of asthma across New York City.

Breathe Easier™ Asthma Management

In partnership with the Asthma and Allergy Foundation of America’s Health Equity Advancement and Leadership (HEAL) program, the Breathe Easier ™ Asthma Management Project (BEAM) addresses the needs of children and adults with asthma in underserved communities of the Los Angeles and San Bernardino Counties that bear some of the heaviest burden of asthma. The Breathe Easier ™ Asthma Management Project (BEAM) provides at-home services to adults with Federally Qualified Health Centers and local providers to provide culturally appropriate case management for individuals with asthma.

The Respira Sano Program is a PCORI (Patient-Centered Outcomes Research Institute) funded asthma comparative effectiveness research project that aims to improve asthma control among Latino children ages six to seventeen in Imperial County, California. Working in partnership with academic/research institute (San Diego State University and the Institute for Behavioral and Community Health) with over 30 years of experience conducting rigorous public health research, a community clinic (Clínicas de Salud del Pueblo, Inc.) with excellent reach and credibility in the partner community, a community-based organization (Comité Cívico del Valle) with extensive experience in promoting asthma control, and a division of the state health department (California Breathing) responsible for asthma control efforts statewide.
