The Hawaii Asthma Initiative is a broad-based, multi organizational, statewide community collaborative whose goal is to bring the public and private sectors together in an effort to increase the quality and years of healthy life for people impacted by asthma, and to eliminate health disparities in Hawaii.

The Hawaii Asthma Initiative is a broad-based, multi organizational, statewide community collaborative whose goal is to bring the public and private sectors together in an effort to increase the quality and years of healthy life for people impacted by asthma, and to eliminate health disparities in Hawaii. This initiative was established in September 2002 and is supported by the Hawaii State Department of Health's Asthma Control Program.

The King County Asthma Program provides home-based education and support intervention for reducing asthma morbidity and urgent health service utilization among low-income and ethnically diverse adults and children with asthma in King County.

Current prioties include
1) Improving quality of primary care through learning collaboratives.
2) Facilitating communication between hospitals and emeregency departments and primary care providers at time of discharge.
3) Providing asthma self-management support and help with trigger reduction through home visits by community health workers.
4) Seeking stable funding for home visits through Medicaid and health plan resources.
5) Promoting implementation of medication self-carry policies in schools.
6) Working with local public housing agencies to assure suitable accomodations
7) Working with local housing permiting and planning agencies to develop certification for "green and healthy" housing.

The mission of the Stanislaus County Asthma Coalition is to create an "asthma friendly" community by promoting awareness, education, management, and prevention.

The mission of the Stanislaus County Asthma Coalition is to create an "asthma friendly" community by promoting awareness, education, management, and prevention.

The Stanislaus County Asthma Coalition is composed of approximately 45 local agencies and organizations. Members volunteer their time to meet and strategize approaches to making Stanislaus County an "asthma friendly" community by looking into the following areas:

1. Public Education
2. Treatment & Management
3. Data & Epidemiology
4. Strategic Planning & Policy Advocacy
