Case Manaagement and educaiton program with the goal to reduce pediatric emergency visits and school absences for asthma in minority communities (Trenton, NJ) by reducing exposure to asthma triggers and by improving ability to self manage asthma symptoms.

Case Management and educaiton program with the goal to reduce pediatric emergency visits and school absences for asthma in minority communities (Trenton, NJ) by reducing exposure to asthma triggers and by improving ability to self manage asthma symptoms.

This is done with a combination of education program in the community. A series of 4 sessions builds on information to develop parental and child confidence in managing asthma symptoms.

Case Management is done on a family basis that assesses client needs, develop individualized treatment plan and goals, developement of personalized Asthma Treatment Plans and encouragement of primary care physician utilization.

Works with community partnerships to identify chldren with frequent asthma related school absences and/or asthma related emergency department visits.

Healthy-n-Green, LLC has several educational materials to educate about good indoor air quality. We work with non-profits, the private sector as well as educators. We provide quality, fun, interactive materials to educate on prevention of asthma attacks through healthy choices in our indoor environments.

Healthy-n-Green, LLC is commited to educating our community about good indoor air quality.  Through educational eco-coloring books and interactive teen books, we have developed materials to educate about not only the problems, but the solutions to good indoor air quality.  

 "Healthy-n-Green is... Healthy Indoor Air" is an educational eco-coloring book for students K-5.  Through illustrations, students will see the "problem" and the "solutions" of indoor environmental triggers.  Resources and tips for parent and/or teacher are at the bottom of each page.  A 9-part teaching curriculum that coordinates with the book is available for school orders.  This teaching curriculum is packed with teaching points, interactive lessons and assessments per section. 

  This fun learning tool will empower our youth to be solution oriented when faced with daily decisions.  They will learn solutions to real life problems and in turn learn how to promote a healthier lifestyle.

Our philosophy is to provide:

  1. Competent, correct and specific asthma care in the form of evaluations and asthma action plans in order to reduce the disparaging effects of asthma.
  2. To assist patients in taking more responsibility and control in the management of their disease as well as reduce hospital admissions and emergency room visits.
  3. To assist patients in working with their primary care providers and specialists in following a plan that is achievable, affordable, and satisfies both physician as well as patient.

Asthma is one of the most controllable diseases of today. Having to go to the Emergency room, Clinic, or be admitted to the hospital is not being in control. Working with your doctor and an asthma educator can help you lead a more active and productive life as well as saving money and time waiting to be seen.
