Centene Corporation provides expansive programs and related specialty services to those receiving benefits under Medicaid for more than 1.2 Million Members. Our asthma disease management program, NURTUR, utilizes a people-centered health management model. With proven experience integrating with other case managers, community resources, and being an active part of medical home, NURTUR is able to provide optimal health management for all asthma patients throughout their continuum of care. Using respiratory therapists as health coaches, patients are taught about compliance, environmental factors and how to follow an asthma action plan. Demonstrated outcomes include decreased inpatient admissions and ER visits for asthma and improved HEDIS measures.

The Monroe Plan Improving Asthma Care for Children program targets high-risk children enrolled in Medicaid and State Children's Health Insurance Programs under managed care to improve the health and quality of life for children with asthma, develop and sustain partnerships with key community stakeholders, and institutionalize innovative clinical models in asthma care.

The Monroe Plan Improving Asthma Care for Children program was designed and implemented using a creative approach to addressing improved asthma care management. The Rochester, New York-based plans targeted high-risk children enrolled in Medicaid and State Children's Health Insurance Programs (SCHIP) under managed care. The initiative seeks to improve the health and health-related quality of life for children with asthma, to develop and sustain partnerships with key community stakeholders, and to institutionalize innovative clinical models in asthma care.
