Since 2000, SUHI and Sinai Children’s Hospital have implemented a series of six comprehensive interventions aimed to decrease asthma-related morbidity and improve the quality of life of inner-city children with asthma and their families.

The Emigrant Trails Tool Kit is a self care card provided free of charge to all individuals who request it.

Seneca County Head Start provides quality, comprehensive services to pre-school aged children and their families, providing information and education about proper nutrition, immunizations, well child visits, and environmental health.

Mid City Community Clinic Pediatrics is a comprehensive asthma program for children ages 0-18 years in a community health center setting that provides a medical home for asthma patients along with case-management and in-home education.

The Asthma and Allergy Foundation of America, Michigan Chapter provides accurate and reliable information and programs to assist patients, families, schools, health care staff, and employers better understand the nature, scope, treatment, and course of asthma and allergies through formal education and training, informal lectures, and community outreach.

Luzerne County Asthma Coalition comprises representation from the hospital/clinic system, medical providers, nurses, health plans, and others to implement a pilot project whereby Asthma Care Packages are distributed at clinic sites to newly diagnosed pediatric patients.
