Respira Sano/Breathe Healthy is a PCORI-funded, asthma comparative effectiveness research project aiming to improve asthma control among Latino children in Imperial County, California.

Genesis TPM’s Asthma Action Plan is patient centric interactive evidenced based chronic care disease portal; where patients, caregivers and schools are collaboratively linked, delivering a tool for the management of Asthma.& COPD .

The Community Asthma Management Program (CAMP) is a collaborative formed by Our Lady of the Lake RMC. OLOL physicians, nurses, social workers and respiratory therapist have partnered with EBR school system and community physicians to provide comprehensive, disease specific resources and education to asthmatic patients and their families.

We are School Based Health Centers serving Cincinnati Public School students and their families with primary care in the school setting. We work in collaboration with the Cincinnati Public Schools Community Learning Centers.

We are working on implementing a program for the 2014-2015 school year.

The office of Coordinated School Health works with local schools to address the needs of the whole child with increasing focus on asthma related issues (absences, etc.).

Our mission is to establish a “partnership” with the medical community to support and enhance the home management of chronic pulmonary conditions.

Working to develop a program specific to asthma medication management.
