Our asthma program provides the highest quality standardized education provided by our interprofessional team to care for the needs of our children and adults in our community.

The ACES (Asthma Care and Environmental Strategies) program is collaborative approach between the WI Dept of Health Services-Division of Public Health and CHW to provide in-home analysis of environmental triggers to asthma combined with a comprehensive client educational program to address pathophysiology, medications, symptoms and early warning signs of asthma, asthma triggers inside and outside the home, and vaccination promotion.

Vitality Books published and distributes a children's picture book entitled "Alfies Attack! The Story of a Fish with Asthma."

18 School based health centers in 5 communities in southeastern Connecticut. Provide primary care, urgent care services in elementary to high school. Provide individual, family and group asthma education.

CDS travels to local physician's offices on scheduled days to provide the convenience and cost savings of local specialized care at the GP level. We provide all materials, equipment/ supplies, and RRT to spend the one on one time that providers aren't afforded, thus decreasing the burden on the clinic's nursing staff.
