I am a Community Health Worker who visits homes of asthma patients ages 5-11yrs. old.

Community Clinic, Inc. (CCI) is a nonprofit, community-based health care agency serving residents of Montgomery and Prince George's counties who are uninsured or under-insured and others. CCI welcomes adults and children in need of primary health care and WIC nutrition education and food supplement services, provided in six accessible locations. Rural Community Asthma Education Program

St. Louis is one of the states that has increased asthma rates for children. The St. Louis City Health Department wants to educate the community about asthma and increase the number of children who take their medicine and asthma actions plan to school, sports activities and childcare facilities.

CHAMP is a CMS Innovations project that involves implementing an asthma registry, providing community based education in homes and schools, and collaboration with community partners.

Teen Asthma Program

The HIPPY for Healthy Homes is a collaborative effort between the UAMS College of Public Health and the children community and outreach organization Home School Instruction for Parents of Pre-School Youngsters (HIPPY) to build capacity in the State for education and health protection from hazards in the home.
